Shepherd’s Cove staff go the extra mile for our patients and families every day. Whether it is providing physical relief from pain, teaching caregivers what to expect and how to care for their loved one, fulfilling last wishes, or helping patients and families celebrate special moments of life, we simply want to make every day the very best it can be.
We’re available 24 hours per day, 7 days a week to help meet the unique needs of each individual patient.
Levels of Care
Shepherd’s Cove offers four levels of care to anyone facing a life-limiting illness.
- Routine Homecare – Routine care is provided inside the patient’s home or long-term care facility. This allows the patient to stay in their home while we coordinate care based on a care plan designed to meet their personal goals. Shepherd’s Cove coordinates and provides medications, equipment, and treatments related to the patient’s hospice diagnosis and provides medical exams in the home, meaning fewer doctor and hospital visits and less stress for caregivers.
- Respite Care – Shepherd’s Cove Hospice patients can stay in the inpatient unit in Albertville for up to five days to provide relief for caregivers.
- General Inpatient Care – This is an intensive, short-term level of care provided at the Shepherd’s Cove Inpatient Unit. GIP care provides a more intense level of care to patients with symptoms that cannot be managed in the home or who need a higher level of monitoring. Once the patient’s symptoms are under control, they will return to their home or permanent living facility under routine care.
- Continuous Care – This is a level of care within hospice that is provided when a patient needs crisis care to achieve palliative (relief from symptoms) or management of acute medical symptoms. This level of care is beneficial to enable the patient to stay at home rather than go to an emergency facility.
